Archdiocesan High School Application Form

Students interested in attending a Catholic high school should download the attached Archdiocesan High School form, complete it, and return it to the school currently attend. The application is due back to your current school by Wednesday, November 19, 2008.

Students attending a non-Catholic private or public school, this form must be submitted to the high school of choice along with transcripts from the previous four years, standardized test results, and the first quarter report card for the current grade by Friday, November 21, 2008.
